Accident Investigation
Accident investigation is a systematic process of examining workplace incidents and accidents to identify their root causes, contributing factors, and circumstances, with the goal of preventing similar incidents in the future. The investigation aims to understand what went wrong, why it happened, and how to implement corrective measures to improve safety and prevent reoccurrence.
Course Aims & Objectives
- Understand the principle of incident investigation and causation.
- Establish a uniform, structured and consistent method for incident investigation, classification, analysis and follow-up.
- Provide participants with the framework and skills required to conduct incident investigation, analysis of incident at summary investigation level, interview witnesses, gather
- Define the terms incident, accident, and near miss.
- Explain why we investigate incidents.
- Explain the incidents causation theory and its use in incident analysis.
- Explain the use and application of the loss causation theory during investigation, analysis and fact finding, and the latent failure(s) or root causes.
- Explain the initial actions to be taken when an incident occurs.
- Define the elements of the investigation technique: gathering information, integrate evidence, analysis facts, draw conclusions, make recommendations.
- Explain the requirements of corrective actions.
